Silicone vs saline breast implants

Your breast implant options

Depending on the look and feel you prefer, there are several options for the type of breast implants to use in your surgery.

  • Silicone implants: These implants are filled with silicone for a natural look and feel that comes in a variety of shapes (round or teardrop), sizes, and gel consistencies.
  • Saline implants: Filled with saline, these implants offer a firmer look with unique volume customization to achieve the perfect cup size
  • Form-stable, “gummy bear” implants: These unique form-stable silicone implants may be a good option for patients with very thin breast tissue or who would otherwise benefit from a shaped implant.

“So, a lot of people get confused as far as whether to get saline or silicone, and the big reason is there’s a lot of marketing, mostly to silicone implants. It’s thought to be a premium product, and I don’t necessarily believe that it is. This is why when we talk about breast implants, there are three main things that you want to look at.

One is look. Can you look at someone and know whether they have saline, silicone, or anything else filling up their implants? The answer to that is no, you can’t.

Can you feel the difference between saline and silicone? So that’s a second thing, feel, and that depends on how much breast tissue they have. If they have very little natural breast tissue, you’re going to be able to feel everything, and if they have a lot of breast tissue, you’re probably not going to be able to feel much of anything inside. Either way.

The third is what’s maintenance like after you get the implant. So did you just buy something that’s requiring you to do a lot, or do you have to do nothing at all? This is where saline really shines. It shines because you don’t have to do anything. If you get saline implants, you just keep going until one day they break, because all implants break.

If you get silicone implants, this is where you have to go get MRIs every 5 years to monitor whether they’re broken or not. So when you’re really looking at whether you should get saline or silicone implants, part of it is saying, “Do you want to do the maintenance?” The other is really taking a hard look at yourself and figuring out, do you have enough breast tissue to hide your implant? If you do, it probably doesn’t matter. And if you have very little breast tissue, you’re probably going to figure out what’s underneath, and you may want to slant towards silicone implants.”–Dr. Samir Pancholi

One of the big things to realize is that there are actually sizes that will fit your body, and that’s why we take measurements. We take measurements of all the different dimensions of your breast, and we’ll have you try them on.

Now, the secret to all of this is that I’m not letting you try on anything you want. I’m letting you try on things that are going to fit you. And that’s where your decision really comes in: which one of these implants matches up with the way you want to look. Even though we may talk about different cup sizes, all of those are just ways for me to understand what you perceive as things that will make your body look good. Once we figure out what that is with implants, we’ve already matched things up. There’s no real need to get into profile implant sizes, CC’s, and everything else. This is a visual thing because after surgery, you’re probably not going to care how many cc’s you have. You’re going to care about what you look like.”–Dr. Samir Pancholi

“Whenever you’re trying on implants, what should you really be looking for to see if this quote fits you or not? The main things that we like to look at are your shoulders. If you feel that you have broad shoulders, you may want to go with implants that are a little bit wider. When they go a little bit wider, they will bring your shoulders in, make your waist small, and make the rest of you just line up and match.

When you look from the side, you want to figure out how you want to look compared to your backside. If you find that you have a small backside, you probably don’t want to go with a very large implant that projects because that’s going to make your butt look even smaller.

On the other hand, the biggest thing is to figure out what proportion you want to have. As you go through the different implant sizes, there’s no need to tell me. The need is for you to look in the mirror and see, does this match up with what you want? And that’s really how you’re figuring it out.”–Dr. Samir Pancholi

Incision placement for breast implants

Breast augmentation is a relatively straightforward surgery for most of our patients. Using the latest scar-minimizing techniques, the procedure takes about 1 hour, and Dr. Pancholi only works with board certified anesthesiologists for your comfort and safety.

There are three types of incisions that may be used during a breast augmentation surgery: transaxillary (or endoscopic.), inframammary, or periareolar. The placement of the incision also reflects where any scarring would appear.

  • Transaxillary: This incision is discreetly hidden in the highest part of your armpit, why it’s often called a “scarless” breast augmentation.
  • Periareolar: This placement of  the incision is along the lower border of the areola, the darker area of skin around the nipple.
  • Inframammary: This incision is placed in the crease under your breast, in the inframammary fold.

The decision about which incision placement is best for you typically depends on your preference, your body and/or your implant choice.

Using the Keller Funnel for Silicone Breast Augmentation

For breast augmentation using silicone implants, Dr. Pancholi uses the Keller Funnel, a device that allows him to place the implant quickly and precisely with minimal contact and minimal trauma to surrounding tissues. This results in a faster, more comfortable recovery and further reduces the risk of capsular contracture and infection to the breast.

Implant placement: under or over the muscle

In addition to the incision, the placement of the implants is also unique to each patients’ goals, body, and implant choice.

  • Over-the-muscle (subglandular): This placement positions the implant beneath the natural breast tissue and over the pectoral muscle putting it under the glands of the breast.
  • Under-the-muscle (submuscular): Implants are placed partially under the chest muscle.
  • Enhanced breast size and fullness by adding noticeable volume
  • Improved breast symmetry by balancing the size and shape of the breasts
  • Restoration of breast volume post-pregnancy or weight loss

  • Anesthesia: Types and methods used during breast augmentation surgery.

  • Implant Profile: Describes the degree of forward projection off the chest wall.
  • Inframammary Incision: A surgical cut made in the crease under the breast.
  • Mammography: A special type of imaging used to detect abnormal changes in the breast tissue.
  • Mastopexy: Also known as a breast lift, often performed in conjunction with augmentation.
  • Periareolar Incision: An incision made around the nipple area.
  • Rippling: Visible or palpable wrinkling of the implant seen on the skin surface.
  • Rupture: A tear or hole in the implant’s outer shell.
  • Seroma: A collection of fluid that builds up under the surface of the skin.
  • Silicone Leakage: When the silicone from a silicone gel implant leaks into the body.
  • Subglandular Placement: Positioning the implant above the pectoral muscle, directly behind the breast tissue.
  • Submuscular Placement: Positioning the implant beneath the pectoral muscle.
  • Tissue Expander: A temporary implant designed to stretch the skin and muscle.
  • Transaxillary Incision: An incision made in the armpit area.

How long does recovery take after an augmentation procedure?

You might be surprised to learn just how smooth your recovery from your breast augmentation can be. If you have a desk job, you can probably return to work in less than a week and you’ll be feeling up to most daily activities in about 2 weeks.

You will be able to see the initial results immediately after your surgery, though there will be some swelling at first. Your implants will settle to their optimal placement after a few weeks following surgery, with your final results able to be experienced after a few months. “One of the big questions is, how soon can I get back to work and doing everything that I want to do after a breast augmentation?

Think of breast augmentation like your first four to five days feeling like you hired a personal trainer, and all they did with you was push-ups. So your chest muscles just hurt. That’s your first four to five days. After that, it all depends on you. How big of an implant did you pick? If you picked a large implant and you stretched your muscle out a lot, it’s going to be a bit more sore. If your muscles are weak, this is the first time that’s in your benefit because your muscles will stretch out quickly, and it’ll stop hurting. But if, on the other hand, you have super strong muscles, they’re not going to stretch as easily, and it’s going to hurt a little bit longer.

After we get past these first 5 days or so, everyone varies little by little. We’re really worrying about whether your implants will drop to where they need to be. Once they drop, then you can start getting back into more and more activity because we know you won’t contract your chest muscles. Everything will fall where it needs to be, and now you can do what you want.”–Dr. Samir Pancholi
